Do I need to show my HEALTH CARD and/or PHOTO ID?
For registration purposes, new patients will be asked to show their BC Services Card (or out-of-province health card), as well as their photo ID (if applicable). If you are registering via email, we will also ask for a selfie photo of yourself holding your photo ID (if applicable), for verification purposes.
What if I do not have a HEALTH CARD?
Patients who do not have a valid BC Care Card (or out-of-province) will be required to pay an uninsured/private fee. You may request a receipt of payment.
What are your PAYMENT METHODS for any fees associated with my visit?
Any fees associated with your IN-PERSON VISIT will be payable via exact cash only. Similarly, payment for any IN-PERSON PICK-UP of any documents will be by exact cash only. Any fees associated with a telehealth (phone) appointment, however, will have the option to pay via Interac e-transfer (unless you are coming in to pick up something following the appointment).
Exception: any fees associated with appointments with our doctor-on-call will be payable by PayPal or exact cash only.
What if I have to CANCEL OR RESCHEDULE my appointment?
If you need to cancel or reschedule your appointment, we kindly ask that you provide at least 24 hours’ notice whenever possible, or as much notice as you can. This allows us to offer your appointment time to another patient. Failure to provide adequate notice may result in a late cancellation or no-show fee. We recommend that you email your cancellation or rescheduling request to welcomeappointments@gmail.com. For appointment cancellations ONLY; you may also leave a message on our alternate line/after hours line: 604-771-1081. This line is NOT to be used for general inquiries or rescheduling.
How can I renew my PRESCRIPTION?
Please book an appointment to discuss any prescription refill requests. If for any reason you are unable to secure an appointment before you run out of your medication, you may contact your pharmacy to see if they can provide you with an emergency supply until your next appointment.
How do I request a SPECIALIST REFERRAL?
If you think you may need to see a specialist, please book an appointment with one of our healthcare providers to discuss your medical concern. During your visit, your provider will assess your symptoms, review your medical history, and determine whether a referral to a specialist is medically appropriate. A referral is not automatically provided and is based on your individual healthcare needs.
How do I request a LAB REQUISITION?
Please book an appointment to discuss your medical concern with one of our healthcare providers. During your visit, your provider will assess your condition and determine whether lab testing is medically appropriate.
Can I request my MEDICAL RECORDS?
If you would like your appointment notes for your visit to be forwarded to your family doctor, please request this at the time of the appointment with the doctor you are seeing.
If you would like to request a copy of your medical record or have your medical records transferred to another physician, nurse practitioner, or clinic, we are happy to assist you. Please note that this service is not covered by the BC Medical Services Plan (MSP), so a fee will apply.
